Both camps are on Phuket Island - under 1 hour flight from Bangkok, and they are situated on the south of the island.
Rawai Muay Thai is where I trained, camp has been open for 6-7 years now, Promtehp Muay Thai camp has only been open for a 1 year (ex-trainers from Rawai are at Promtehp). If you go for Promtehp I suggest you book a private session with Yib; great trainer.
They have accomodation at the camp by the way plus it can be a great way of meeting loads of people who like you are looking to have some fun.
I was going to suggest the same thing without the personal touches you added.

I believe that he was talking about getting a Sak Yant Buddist tattoo from a monk at say Wat Bang Phra. In which case Hepatitis is your biggest worry. HIV would have an extremely hard time surviving and probably impossible for it to survive the process, but I will not og into the empidemiology of HIV. If you look at how the process is done you can see how it would be hard for a blood borne pathogen to be transimitted. BUt yes if you go to a local parlor then you want to make sure it is a reputable establishment that uses thermal sterilization techinques using autoclaves or that use completely new sterilized needle packs.

If this a significant worry then I would either not do it or wait till you have a group of tourists large enough to constitute a party so you do not have to get tattooed with locals. As you can see from the Wiki, I would not use this as sole verification, no tourist has reported a case of contracting a blood borne pathogen after recieving one of the tattoos. Also I am fairly sure that I would have heard about it if there was any statistical significance to this individual tattoo worry.
